Home Health Winneshiek Medical Center Home Health provides services to individuals and families in their own homes, offering an alternative to costly long term stays in a hospital or nursing home. Home health care is health care provided to people in their own home. People receive home health care for both chronic conditions, such as Congestive Heart Failure or Diabetes, or for acute medical conditions, such as after a stroke or a surgical procedure. The primary services provided to patients are intermittent skilled nursing visits. Depending on the unique needs of each patient, visits from other health care professionals such as home health aides, physical or occupational therapists, and a social worker are also available. Nurses help with educating patient's on, and monitoring of, their condition. Home health aides provide assistance with personal cares, such as bathing or hair-washing, and may also provide a limited amount of housekeeping assistance. Therapists assist patients with strengthening and with enhancing the safety and accessibility of the patient's home environment. Individual needs are assessed and personal care plans are designed under the direction of the patient's own physician. Nurses are on call 24 hours a day, seven days a week Costs:Winneshiek Medical Center Home Health is a Medicare certified agency. Therefore, Medicare, Medicaid or private insurance can be billed for these services. No one will be denied home health care because of their inability to pay. Charity care may be available. Referrals:Referrals may be made by physicians, nurses, other health care providers, families, friends, clergy or patients themselves.
