Winneshiek Medical Center - Decorah Clinic and Mabel Clinic offers reduced rate athletic physical examinations to area athletes.  To accommodate students participating in winter and spring sports, these physicals are available year-round. 

Athletic physicals are $30, and must be paid by cash, check or credit card at the time of the visit.  Students should bring their Iowa or Minnesota athletic physical forms - completed and signed by parents - to the appointment.  Students not seeing their regular health care provider for the physical should bring their immunization records.  If any immunizations or additional services are required, there will be an extra charge for these services.

If the student is under 18 years of age, and a parent will not be accompanying the student, consent by the parent will need to be given to WMC - Decorah Clinic.  This can be done by phone or a signed note sent with the student.

Optional Musculoskeletal Assessment

An optional musculoskeletal assessment will be provided by the Rehabilitation and Sports Medicine Department at Winneshiek Medical Center, and is included in the $30 athletic physical fee.  The assessment will be performed by the Winneshiek Medical Center athletic trainers and physical therapists, and will take place in the Rehabilitation and Sports Medicine Department at the medical center. 

A musculoskeletal assessment is optional and it will cover the following areas:

• Joint flexibility and strength
• Posture
• Joint stability
• Injury history review
• Information on strength and conditioning
• Nutritional information for the athlete

Please inform the scheduler at Decorah Clinic if the athlete would like to participate in this assessment.
