At Winneshiek Medical Center in Decorah, you will experience personal care for women’s health issues. Our gynecologist, Randall J. Bremner, M.D., FACOG, is a full-time Mayo Clinic Health System specialist at the Clinic in WMC providing daily gynecologic care for women of all ages.
Abnormal uterine bleeding
A hysterectomy is just one of many options available to treat abnormal uterine bleeding. Dr. Bremner will explore nonsurgical solutions first, such as specialized medication and hormone therapy. He also offers an outpatient procedure called endometrial ablation, which is a minimally invasive alternative to hysterectomy. If a hysterectomy is needed, he offers laparoscopic supracervical hysterectomy (LSH), which is minimally invasive and can have a shorter recovery time than traditional surgery.
Over time, the effects of childbirth, injury or the natural aging process can cause your pelvic floor muscles to weaken. Dr. Bremner, along with local Mayo Clinic Health System urologist, Brad Orvis, M.D., offer you the newest treatment methods to reconstruct your pelvic floor and restore you to your normal function.
Drs. Bremner and Orvis also provide a multidimensional approach to treating urinary incontinence (leaking urine), which may include: bladder retraining with WMC’s certified bladder retraining therapist, muscle exercises, diet counseling, biofeedback and medications. They also offer the minimally invasive tension-free vaginal tape (TVT) procedure that supports your bladder to prevent accidental leakage.
Birth control counseling
As a woman, you have unique contraceptive needs and wants. Dr. Bremner offers a level of personal care that you will appreciate. He takes the time to talk with you to hear about what’s going on with your health and your life, and to answer any questions or concerns you may have. In addition, Dr. Bremner offers the most up-to-date birth control options including Mirena IUDs, and in-office placement of permanent, hormone-free birth control.
Abnormal Pap smear
It is important to remember that most cases of abnormal Pap smear results do not indicate cancer. The evaluation of an abnormal Pap smear often involves an office procedure called a colposcopy. Based on the results of the colposcopy, Dr. Bremner will present options for treatment of the abnormal cells. If abnormal tissue is identified, Dr. Bremner can often remove it with the loop electrocautery excision procedure (LEEP), which can be performed as an office procedure.
Dr. Bremner places a major emphasis on menopause health. He will spend time talking with you about your health status, your lifestyle and your goals for the future. Together, you will develop a personalized menopause wellness plan that meets your unique needs.
If you are having trouble conceiving, you may be worried, frustrated and wondering how you’ll make your dream come true. Dr. Bremner will discuss your situation with compassion and sensitivity. He will meet with you and your husband together and go over your health histories to help him create a program based on your individual needs – which may include medications or surgery.
Other health care issues that Dr. Bremner can effectively treat include PMS, STDs and sexual intimacy concerns.

But if surgery is your best option, you can feel confident knowing that Dr. Bremner and the staff at WMC offer the latest, most effective techniques, including laparoscopic surgery – which is a minimally invasive alternative to traditional surgery that generally results in less pain and a quicker recovery and return to regular activities.
